Job Description

Our client, a 140-bed, five-story hospital with advanced technology and patient-focused care located in Temecula, California, is urgently looking for a Heart Valve Clinic Coordinator.


ROLE DESCRIPTION

The Heart Valve Clinic Coordinator will be responsible for receiving and reviewing referrals for appropriateness, reviewing with an attending physician as needed. They will facilitate direct contact between cardiologist and referring physician regarding treatment plan, and participate in development of patient treatment plans. The Coordinator will also be responsible for contacting the patient/family; explaining the procedural process and work-up. In addition, they will review advanced directives and insure conversion decisions are well documented and on the patient chart.


DUTIES AND RESPONSIBILITIES

  • Perform functional assessment and frailty testing; have all testing available for review by cardiologist and CTS at valve clinic visit and coordinate any additional testing i.e. Cath, TEE
  • Upload required data to Valvepoint and maintain communication with clinic
  • Facilitate scheduling and authorization of TAVR procedures
  • Round daily with attending physicians on TAVR patients; coordination regarding time of rounding to be established between attending and TAVR nurse


SKILLS AND CERTIFICATIONS

  • Minimum of 3-5 years of Cardiac related experience or related Heart Valve experience preferred
  • Current CA RN license required
  • BSN preferred
  • BLS (obtained through AHA)
  • ACLS (obtained through AHA)
