JOB DESCRIPTION:
Nurse Casemanager - Homecare
The right candidate will report to the Patient Care Coordinator. The Home Care Casemanager is a clinically competent, registered, professional nurse who is responsible for effectively and efficiently managing a patient caseload to include accurate assessment of patients’ functional, clinical and service utilization. The Casemanager also administers skilled nursing care and medical nursing care to patients requiring intermittent professional nursing services including assessment, planning, intervention and evaluation within the confines of the patient’s home. The Nurse Casemanager also supervises/monitors and provides education to the family/patient caregivers, staff nurses and para-professionals on specific health needs and tasks to promote optimal outcomes.

QUALIFICATIONS:
• BSN
and
5 years of Home Care experience
