Job Description
Overview
Reporting to the Regional Director/Manager of Care Management, is responsible for identifying, assessing, planning, coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The case manager coordinates the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. The case manager works with patient and treatment team to help establish goals and realize optimal outcomes as determined by current individualized care plans.
Qualifications
The job incumbent is required to demonstrate the knowledge and skills necessary to provide patient care appropriate to the age and types of the patients served.
This requires that the incumbent demonstrates knowledge of the principles of growth and development as well as the physical, emotional and psycho social needs of the patient population served.
Work requires the completion of a BSN, or obtain a bachelor’s degree within 5 years.
Current State Registered Nurse license as required.
Work requires 3 to 5 years of progressively more responsible in appropriate clinical area in order to develop proficiencies in delivering high quality patient care services, develop an understanding of work flow of patient care units, and to demonstrate proven leadership skills required by the position.
Work requires substantial interpersonal skills to lead and direct the efforts of a group of professional and nonprofessional staff and to develop and maintain cooperative working relationships with personnel from other departments.
