Job Description

GENERAL PURPOSE:

· Utilize professional nursing theory and the nursing process to implement, coordinate and direct patient care and auxiliary nursing care activities, using a community health center primary care model.

Clinical Assessment

Demonstrates assessment and identification of patient/family health needs.

Assists with assessing the patient to facilitate care planning and identification of problems.

Utilizes data collected and reported by providers, other clinical support staff, patients/families/significant others, other disciplines and community agencies.

Reviews payer sources and requirements that may affect patient care.

Demonstrates the knowledge and skills necessary to provide care appropriate to the patient’s age (i.e. knowledge of growth and development, treatment and care options).

Demonstrates a strong knowledge of nursing practice and procedures.

Demonstrates the ability to teach and assess patient learning, including informational handouts.

1. Provides clear and concise patient education and instruction in understandable terms.

Demonstrates sound knowledge base in age-related care, concepts and ability to provide education to patient/family or staff as needed.

Documents relevant information in the EHR according to MFHC and nursing requirements. 1

Asks for assistance when necessary.

Obtains necessary vital signs and history information from patient/family/guardian.

1. Completes EHR and hard-copy forms as appropriate.

Consistently follows telephone triage procedure, including thorough documentation and notifying provider in a timely manner (if indicated).

Assists with making referrals and assuring process is completed.

C. Clinical Skills

Prepares rooms, equipment and patients for office visit/procedures (i.e. cleaning and setting up exam room/procedure room/equipment, providing patient instruction).

Knowledgeable regarding equipment necessary for various procedures.

Consistently plans ahead to insure availability and readiness of supplies and equipment.

Competently assists providers with confidence and in anticipation of provider needs.

Performs lab tests in accordance with MFHC, state, and safety practices, including infection control and sharps disposal.

Maintains proper documentation in a timely manner.

Notify provider of any abnormal test result in a timely manner.

Assists in maintaining medical records and health center tracking logs.

Knowledgeable of grant programs/payer sources and services covered. Problem solve when issues arise re: covered services or payer sources.

++JOB QUALIFICATIONS++

A. ++Education or Formal Training & Experience:++

a. Registered Nurse with two (2) years’ experience in primary outpatient care preferred.

b. Current Colorado license.

c. Current B.L.S. certification.

d. Bi-lingual in English/Spanish preferred

B. ++Knowledge, Skill & Ability:++

a. Demonstrate excellent communication, assessment and problem-solving skills.

b. Demonstrate strong knowledge in nursing practices and procedures.

c. Demonstrate a strong knowledge of medical equipment, nursing procedures, documentation, patient education and knowledge of community resources.

d. Competent in general office skills such as computers, faxes, telephones and copy machines.

e. Competent in computer skills and EHR/EPM use.

f. Ability to organize and prioritize work load.

g. Maintain effective and positive professional working relationship with staff and patients at all times.

h. Ability to understand and respond effectively and with sensitivity to special populations served by MFHC.

i. Ability to work under pressure in a fast-paced environment.

j. Ability to travel to other locations and to meetings outside of the service area.

k. Ability to work flexible hours to meet job requirements

C. ++Physical Requirements and Workplace Environment:++

a. Job offer contingent on being free of active tuberculosis.

b. Requires the ability to lift up to 30 pounds.

c. Requires ability to hear routine conversations.

d. Requires periods of standing and walking while seeing patients.

e. Requires sufficient near vision to be able to read documents and computer screen.

f. Requires movement from room to room.

g. Requires ability to stoop, kneel, bend, crouch and reach.

h. Requires ability to comprehend both oral and written communications.

i. O.S.H.A. classification Category 1: All procedures or other job-related tasks involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues or a potential for spills or splashes of these fluids.

j. HIPAA Classification: Unrestricted Access: A workforce member with unrestricted access will have full access to patient’s protected health information, including the patient’s entire medical record, for patient care purposes.

k. Must pass a criminal background check

l. Annual Flu vaccine required. Employer will provide

m. Obtain PPD for tuberculosis (annually if provider). Employer will provide.

n. Obtain vaccine records for MMR, Varicella, T-DaP and Hep B. Employer will provide.
