RN- Coordinator of Care
Bronx, NY
Shift: 12 or 10 hr Day Shifts, Rotating Weekends.

Job description:
As the Coordinator of Care RN, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers.

Required skills & experience (the “must haves” to be considered):
NYS RN License
1 year of nursing experience (any type of experience EXCEPT peds/maternity/nicu)
CHHA experience preferred
BSN Preferred
