Description

This position is responsible for promoting optimal care in clinical areas determined on an intermittent basis to be of high priority for performance improvement by the Quality Department and Clinician Directed Performance Improvement (for example, sepsis, stroke, and c. difficile infection). The primary activities for this position will be real-time bedside monitoring and clinician support to ensure compliance with evidence based guidelines and policies. This position will also be responsible for developing systems for ensuring continuity of care practices during times when the Coordinator is not on site, and relevant education for staff in support of performance improvement projects. This position will have some minimal responsibility for data collection and analysis but primarily will be expected to be in clinical areas and engaged in patient care and staff and provider interface. This position acts as a resource to all patient care areas regarding medical and nursing care as related to patients with targeted clinical diagnoses. Manages care by maintaining effective lines of communication with all stakeholders.


Requirements

MINIMUM QUALIFICATIONS:

EDUCATION: Graduate of an accredited school of nursing.

CERTIFICATION/LICENSES: Current New Mexico State or compact state RN license. CCRN certification preferred. BLS issued through the American Heart Association. ACLS issued through American Heart Association and NIHSS required within 6 months of hire/transfer date. Must maintain membership in AHA/ASA and CBRAC.

SKILLS: Demonstrated excellence in communication, interpersonal and analytical skills. Must be computer literate. Must be self-directed. Experience with Excel preferred.

EXPERIENCE: Minimum of three to five years' experience in emergency/critical care nursing, with advanced skills.

NATURE OF SUPERVISION:

-Responsible to: Executive Director of Quality and Medical Director of Clinician Directed Performance Improvement

General office environment and general hospital departments/nursing units

PHYSICAL REQUIREMENTS:

Ability to move around the hospital to all units/departments for about 70% of the day, sitting in office 30% of the day. Ability to assist with patient lifting and general nursing care. Use of a computer for required applications. Requires close work, good vision, dexterity to write as well as to type.
