Under the direction of the Clinical Services Director, the WelbeHealth Clinic Supervisor RN is responsible for organizing and directing the day-to-day work at the clinic, supervising the clinic staff, and coordinating clinic flow to ensure effective WelbeHealth clinical services are provided, and quality of care standards are met. The Clinic Supervisor is the dedicated RN in the clinic and is adept at both people management and RN level nursing care. The Clinic Supervisor is a member of the WelbeHealth leadership team and facilitates collaborative
relationships across WelbeHealth and with external stakeholders to promote the WelbeHealth values, team culture and mission.

Duties and Responsibilities:

  • Provide leadership, oversight, support, guidance, and performance management to all
    Center and Clinic LVNs and MA roles, ensuring all nurses and clinic staff have role
    clarity and the required training to perform their functions at the top of their license.
  • Oversee the overall functioning of the Clinic and clinical needs in the center, to
    also include collaborating with RN Case Management function to ensure chronic and
    acute needs of participants are addressed properly and liaising with Complex Care
    Team on smooth transitions of care to meet post-discharge needs. 
  • Develop staffing plan and ensure appropriate staffing, while recognizing the diverse skill
    levels of individual staff members and the unique needs of the participant population.
  • Effectively intercede with participants and their families and resolve concerns related to
    the delivery of health care services. 
  • Ensure assigned staff adhere to clinic policies, protocols and procedures.

Clinical

  • Develop, maintain, and evaluate participant and staff data collection systems and
    processes to support WelbeHealth’s clinic and the delivery of participant care.  
  • Run PM Huddle and participate in standing Clinic meetings, as assigned. 
  • Ensure delivery of after-hours handoffs to/from Nightly Navigators. 
  • Triage walk-ins directed to Clinic by Center Nurse. 
  • Anticipate and support smooth participant enrollments into the program.  
  • Participate in end-of-life care coordination and support with PCP.
  • Manage onsite medication inventory.  
  • Act as clinic point of contact for questions on clinic activity from the Day Center team. 
  • Ensure PCP support roles are staffed and communicated appropriately to team
    members and providers and ensure rooms are assigned for all planned visits. 
  • As needed, perform direct participant care, operate equipment safely and correctly, and
    carry out medical treatments and special procedures within scope of practice. 
  • Instruct the participant, family and caregivers regarding the disease process, self-care
    techniques, and prevention strategies, as needed. 
  • Perform and document ordered procedures and care, in accordance with scope of
    practice, as directed by PCP. 
  • Support Clinic and Center nurses in destruction and documentation of expired
    medications when indicated by policy.
  • Develop, maintain, and evaluate participant and staff data collection systems and

Administrative

  • Monitor all staff clinical inboxes and address any backlogs or need for additional training.
  • Evaluate staff competencies by direct observation, peer overview, orientation and
    employee input, documenting this in the appropriate system. 
  • Review and sign off on assigned staff competency attestations. 
  • Assist clinic team with Athena questions as Athena Super User. 
  • Ensure assigned staff deliver and document interventions and care in Athena. 
  • Remain alert and update the supervisor or any member of the IDT promptly of any
    changes in participants’ condition. 

Quality/Compliance/Audit related

  • Ensure the clinic complies with WelbeHealth requirements and Federal and State
    regulatory standards. 
  • Monitor clinic operations daily to ensure infection control processes and protocols are
    followed. 
  • Work with Clinic team and quality to implement and evaluate clinical outcome
    improvement plans.
  • Conduct regular audit readiness clinic walk-throughs. 

IDT/Care Planning

  • Participate in standing Clinic meetings such as Morning Briefing/Huddle/IDT/Ad Hoc or
    other attended meetings and complete action items as assigned. 

General

  • Maintain assigned clinical inboxes and manage and respond to Teams communications
    and requests.
  • Manage direct reports including hiring, training, supervising, and mentoring.
  • Be legally authorized to practice in position location, required.
  • Only act within the scope of his or her authority to practice.
  • Complete all required documentation in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information
    about team members, participants, and families.
  • Follow WelbeHealth policies and procedures and participate in any required Quality
    Improvement activities, staff training and meetings.
  • Communicate regularly with Supervisor and team regarding workload and priorities.

Qualifications and Requirements:

  • Graduate of an Accredited School of Nursing:
  • Unencumbered Registered Nurse (RN) License with which you practice required.
  • (BSN) preferred.
  • Valid State Driver’s License with a clean DMV record.
  • Copy of recent Vehicle Insurance required.
  • Minimum of five years’ nursing experience in a clinical setting with a frail or elderly
    population.
  • Nursing knowledge and skills necessary to treat frail, elderly participants and manage
    complex clinical situations.
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment
    and make sound decisions in emergency situations.
  • Excellent clinical, organizational and communication skills in settings with seniors,
    their families and interdisciplinary team members.
  • (2 ) years’ supervisory experience with demonstrated ability to mentor and develop
    team members.
  • Ability to process accurate information in a timely manner.
  • Excellent leadership, organizational and communication skills in settings with seniors,
    their families and interdisciplinary team members.
  • Experience leading in a data-driven organization, leveraging reports and data to
    prioritize and manage people and projects.
  • Highly motivated, inspirational leader who is able to innovatively engage, support and
    remove barriers for the team to ensure the team consistently meet established goals.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to prioritize in a fast-paced environment.
  • Experience and competency working with people from diverse backgrounds and
    cultures.
  • Commitment to unlocking the full potential of our most vulnerable seniors.
  • Bilingual English/Spanish preferred.
