Description
Works with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Assesses and responds to patient/family needs by coordinating efforts of other team members. Identifies and resolves barriers that hinder effective patient care.
Requirements
- Graduate of accredited ADN or BSN program. BSN Preferred
- Basic Computer Knowledge
- Ability to communicate effectively, both verbally and written
- Minimum of five years recent clinical experience as an RN. Negotiating skills which support the ability to interact with physicians, nursing staff, discharge planners, and payers. Excellent verbal and written communication skills, Knowledge of clinical protocol, normative data and health benefit plans, particularly coverage and limitation clauses. Ability to manage several projects and cases simultaneously. Knowledge of community resources.
- High school diploma or equivalent.
- Maintain state licensure and provide organization with verification.
