Overview

Assessment and Intake Nurse (FT)

Triangle Springs is a 77-bed behavioral hospital located in Raleigh, NC. The hospital provides a full continuum of care for those suffering from a mental health issue and/or chemical dependency. This includes inpatient treatment, partial day programming and intensive outpatient services. Assessment services are provided 24 hours a day, seven days a week. Position Details

  • Evaluate incoming patients to determine the severity of their illnesses and the intensity of services they will require while in the hospital.
  • Collect medical-history information, medication list, and other pertinent information related to the patient’s course of treatment.
  • Implement and use current standards to prevent spread of infection.
  • Instruct and prepare medications as appropriate for the patient's care and safety.
  • Triage with other team members of the Assessment and Intake Department, as well as contact on-call providers and obtain admission orders to be inputted into an electronic medical health system.
  • Monitor and review referrals received from emergency rooms, local agencies, and various community providers to determine criteria for certain levels of care.

Qualifications

Requirements:

  • Graduate from an accredited program of professional nursing training
  • Current unencumbered license to practice by the State Board of Nursing
  • CPR Certification and Crisis Prevention Training (CPI) within 30 days of employment and prior to any patient contact

Preferred Experience:

Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred. Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities.
