Day, evening, or night shift available.

Job Summary:
Under the direction of the registered nurse, provides nursing care in accordance with departmental and hospital policies and procedures, supporting the care plan established for each resident.

Job Qualifications:
- Completion of an LPN program
- Previous long term care experience preferred
- Ability to communicate effectively and document information accurately
- Ability to deal efficiently and effectively with a wide variety of individuals
- Knowledge of medical terminology
- Ability to move or lift residents and equipment
- NYS license to practice as an LPN
- CPR Certification required

Duties include:
Transfers, moves and repositions clients every 2-3 hours and prn.
Assists clients to move to activities area or dining area, etc.
Keeps unit neat, relaxed and homelike.
Assists with admissions, transfers and discharges.
Provides treatments/medications as ordered.
Operates all equipment and performs all procedures/care in a safe manner.
Notifies appropriate persons/family members of significant events (in conjunction with RN).
Completes work assignments on time/readily accepts assignments as observed by RN.
