Why This Role is Exciting: Lead a department of dedicated RN and Social Worker Case Managers. Drive quality care and discharge planning in a 70-bed rehabilitation hospital. Collaborate with leadership and external partners to ensure the best patient outcomes. What You'll Do: Oversee the daily operations and staff management of the Case Management department. Guide interdisciplinary care planning and discharge processes for optimal patient results. Serve as a patient and family advocate while building strong relationships with payers and healthcare partners. What You'll Bring: Active RN license in Florida and BSN or higher. At least 3 years of hospital-based Case Management (acute care, inpatient rehab, or LTACH). 2+ years of leadership experience (supervisor level or above). Strong skills in communication, organization, and collaboration. What's In It For You: Salary: $95,000 - $110,000 annually (based on experience).
