Registered Nurse (RN) - Medical Home Port Job Overview:

  • Assess each patient and perform triage duties as assigned.
  • Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
  • Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.
  • Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
  • Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
  • Assist in planning, provide clinical direction and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel.
  • Communicate with patients utilizing Secure Messaging to improve communication and facilitate care through non-traditional means.
  • Utilize available automated programs and information technology, communication and management tools for proactive patient management and to facilitate patient engagement and enhance patient experience (i.e., AHLTA, TSWF, CHCS, CarePoint and Secure Messaging).
  • Perform a full range of RN duties in accordance with assignment under the task order, including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians' orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.
  • Ensure upkeep and scheduled performance checks of emergency equipment i.e., oxygen, AED and pads, anaphylaxis kits, emergency cart, suction apparatus, etc. and maintain appropriate logs. Inform the supervisor of any discrepancy on daily checks and ensure prompt corrective action.
  • May administer the administration of routine intravenous fluids, vaccines and/or medications.
  • Initiates and maintains medical history on patients, recording observations and stated symptoms for use by the MCMH Team. Observes and records pertinent vital signs, determines and orders laboratory procedures on patients that may be required by the provider to diagnose and treat the patient, per command and department policies.
  • Assist providers with physical examinations, treatments and diagnostic procedures.
  • Coordinate consult/referral/follow-up/tracking process of hospitalizations, ER visits and ancillary testing.
  • Plans, coordinates, implements and evaluates activities related to the delivery of ambulatory nursing care within MCMH.

Registered Nurse (RN) - Medical Home Port Job Qualifications:

  • BLS
  • Computer Competency
  • Graduation from an accredited registered nurse program
  • RN Licensure: current unrestricted license, any U.S. jurisdiction.
  • Minimum of 1 year of full-time clinical experience within the last 2 years as a RN.
