RN-Nursing Supervisor

We are looking for an experienced Nursing Suervisor in Vallejo, CA. This will be 3 x12 shifts, and it is primarily night shift 7 pm- 7a but could be some day shifts if folks take PTO. Also, will need to be able to float to either ED or Med Surg.

Duties:

Bed Access Coordinator serves as a clinical and professional role model. Effectively coordinates resources using the multidisciplinary approach; planning, directing, implementing and improving service while supporting the MHLB mission and goals. Will be required to work flexible hours to efficiently manage and communicate with staff on all shifts. Is responsible for the overall coordination of patient care in hospital when Managers are absent. This includes staffing and administrative concerns that arise during an assigned shift.

1. Functions as the central point for all admissions and transfer bed assignments utilizing the appropriate policies, (not limited to but including admit, discharge and transfer criteria).
2. Facilitates bed huddles at 0800and 1500 daily Communicates bed availability to the ER by 0830 and 1530 daily.
3. Responsible for ensuring appropriate physical placement of beds.
4. Update bed map with changes as they happen.
5. Screens all admissions and transfers for appropriate bed assignment using Inter-Qual criteria, physician plan of treatment and use of critical thinking skills. Personally reviews charts.
6. Documents severity of illness and intensity of service in the Midas Case Management work list from 1600 to 0730 daily, all day Saturday and Sunday and on holidays for all new admissions. (anytime discharge planning is not here) on the discharge planning in not here.)
7. Reviews observation charts prior to 24 hours for upgrade to inpatient status or discharge. Reviewing criteria as appropriate. Obtain physician order for status change.
8. Accountable for managing all variances to patient bed assignment including nursing staff, blocked beds and schedule hospital services.
9. Clearly communicates hand off to oncoming shift related to current bed status and variances.
10. Refers to administration on call when census gets close to maximum capacity.
11. Will use the Bed Priority Policy when assigning beds.
12. Responsible for time patient flow from one department to another. Early notification of anticipated admission from the ER is necessary for timely bed planning. With target time from ER to inpatient being physically placed on unit is less than one hour


Skills:

Will be proficient at Inter-Qual and will closely monitor all admission, either inpatient or observation status.

MINIMUM QUALIFICATIONS:
1. Current licensure as a registered nurse in the State of California.

2. Three years (3) clinical nursing experience with one year in the area of clinical concentration.

3. Demonstrated leadership, communication and problem solving skills.

4. Ability to write, read, hear and verbally communicate in English to the degree required to perform the job.
