Job Description
Client in Columbus WI is seeking a permanent, full-time, RN Case Manager to join their Home Health team!
* Competitive hourly compensation
* Full benefits package
* RN must have at least 2 years of recent Home Health or Hospice experience
* WI or Compact RN License and BLS required
* Position is M-F Days, 8a-4:30p, with rotating weekends, and some on-call
* RN is required to make a minimum of 25 visits per week; 5 per day
* Must have reliable transportation, vehicle insurance, and a valid drivers' license
