Home Health Case Manager
- Shifts = Monday-Friday 8a-5p
- At least 1yr Home Health experience required
- Oasis experience required
- EMR = Kinnser
- We prefer applicants who have at least 2 to 3 years of diverse nursing experience, including experience with infusion and wound care.
- We will consider candidates who do not possess all of these skills if they provide excellent patient care and are eager to learn

Requirements
- Infusion experience strongly recommended for this position.
- OASIS fluency, infusion, and wound care preferred
- You must have 2-3 years of diverse nursing experience to be successful in this role
Applicant’s Must Have:
- A valid driver’s license
- Car insurance
- CPR card
- Current professional License
- A TB test will be provided
