RN – Care Services Nurse – PRN - Care Services (Admissions)
Completes process to admit patients in the home or hospital and coordinates care of hospice patients admitted to area hospitals. Computer skills/experience and nursing assessment and care coordination experience required. Hospice and Hospital experience preferred. Minimum 1 year nursing experience required. Must be proficient at medical record review and have excellent verbal communication skills. Flexibility and willingness required to assess patients in their homes or in local hospitals when needed. Must be willing to be trained and float to work other Hope Service Areas to meet organizational needs. PRN – scheduled on an as needed basis - Must be available to work a minimum of 2 days per week (Mon-Fri) - including a minimum of 2 weekend days per month. .
