Job Summary

Carry out supervisory responsibilities in accordance with Hope’s Leadership Guidelines. Assume responsibility for continuity, appropriateness, and quality of service while providing compassionate care, uncompromising service and excellent communication with patients and families. • Key Responsibilities
  • Supervision of CNA, LPN, and RN staff.
  • Triage admission process for referrals and urgent care issues.
  • Staffing assignments.
  • Initiate / facilitate the management of complex cases with appropriate Senior Manager.
  • Recertification / decertification & discharge.
  • Supervise the patient care activities of the interdisciplinary team.
  • Monitor documentation & implementation of the careplan.
  • Collaborate effectively with the Director, Hospice Care.
  • Communicate successfully with other Hope Healthcare services.

• Knowledge & Skills
  • Hospice or home health
  • Adult Health, Primary Care and / or Geriatrics
  • Leadership and management of staff

• Required qualifications
  • RN Required – Licensed in FL, BSN strongly encouraged
  • Experience as a manager in a hospice or home health setting required
  • Hospital or skilled care experience desired
  • Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation.
  • Clear level 2 background check
