Job Description

MedCare Staffing is seeking an experienced and compassionate Hospice Nurse for an exciting opportunity in Dallas, TX. In Roles include nursing assessments, maintaining patients' comfort during the final stages of life, and providing loved ones with guidance and support. The hospice nurse’s main responsibility is to build, execute, and manage the plan of care for each of their patients. Will report to the Nurse Team Manager or Director of Clinical Services with any observations, comments, or concerns related to their patients.

This position will start out as part time and eventually move to full time.

Job Duties:

-Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
-Preparing and adjusting patient care plans as needed.
-Frequently visiting patients to monitor vital signs and physical comfort levels.
-Performing prescribed treatments and administering pain management medications.
-Informing physicians about changes in patients' conditions.
-Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
-Maintaining and updating medical charts, patient files, and health records.
-Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
-Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
-Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service provided.
-Prepare clinical and progress notes that demonstrate progress toward established goals.
-Perform on-call responsibilities and provide on-call service to patients and families as assigned

Education/Experience
-ADN/BSN
-Texas RN license
-2 years of Hospice experience preferred
