Job Description
Large, established NYC home care organization looking for RNs to work on a full-time basis in a Nurse-Family Partnership program. RNs in this position will partner closely with mothers and their newborns from birth until the 3 years of age. Working to break the cycles of poverty and lack of access, RNs will provide post-partum assessments; newborn assessments; support for post-partum issues such as lactation and post-partum depression; patient education for both the parent and the child; and collaborative care as part of the patient's care team. This is a hybrid position, in which RNs conduct both in-home and telehealth visits.
What this position offers:
- Competitive salary range between $93,000 - $112,000/year

- 2+ year(s) of direct experience
- RN-BSN's highly preferred
- Experience in post-partum and early childhood care; ante- and peri-partum experience also eligible for consideration
- Home care experience (especially in maternal/child health) highly valued
- Driver's license + personal car
