Job Description
Seeking a Population Health Community Health Worker to work with assigned primary care departments and the community health department to focus on improving health equity, improve health outcomes, and promote well-being of our patients by closing gaps in care through patient engagement, barrier identification and care coordination. They will work to ensure that the primary care departments have the support necessary to ensure patients are up to date on priority chronic disease care and preventive care.
To improve our patients’ health and wellbeing, the CHC’s primary care patients are screened for both medical and social determinants of health (SDOH). CHWs work with a panel of patients who have screened positive for SDOH needs and/or are identified for chronic disease health coaching.
Schedule: Monday-Friday 8:30-5:00pm
Education:
· High school diploma or GED required.
· Relevant prior work experience, OR Associate or bachelor's degree preferred.
· Training from a CHW program preferred.
· Portuguese/Kiswahili/ Khmer/ Arabic/Spanish/ Vietnamese fluency highly desirable.
· Reliable transportation required; Driver's License (Preferred).
· Proficiency in Microsoft Office (Excel, Word, Outlook, and PowerPoint) and online record keeping for work-based communication.
