Job Description
Seeking a Community Health Worker for a diverse, community-based health care organization.
The CHW will provide education, engagement, and long term follow up of those at risk of acquiring HIV through drug using or sexual partners of individuals with known HIV+ status or unknown HIV status. Monitor lab results needed to continue on PrEP, and support patients who are on PrEP at the Health Centyer per the standards and guidance of the Massachusetts Department of Public Health and the Office of HIV/AIDS (MDPH/OHA). Facilitates access to PEP (post exposure prophylaxis) as needed. Provide outreach/in-reach and recruitment as regulated by MDPH/OHA. (Includes offsite outreach).
Education:
- High School diploma or equivalent.
- One year of community health/public health experience preferred.
- Proficiency in phlebotomy preferred.
- Ability to read and write in English required. Bilingual Spanish/Khmer/Portuguese preferred.
