Compensation Package:

Total compensation ranging from $137-147K annually ($65-70/hr)

Responsibilities:
Diagnose and treat acute, episodic or chronic illnesses
Performs medical assessment, procedures, and determines treatment recommendations
Facilitate referrals to other healthcare professionals and medical facilities
Communicate with collaborating physician or specialist regarding patient care
Provides extreme customer service to patients, visitors and coworkers
Directs and reviews all laboratory and radiology results and develops appropriate treatment plan
Provides patient counseling, education and coordination of care as necessary
Performs other duties as assigned
Qualifications:

Required Qualifications:
Current, unrestricted licensure in the State of Arizona, or ability to obtain prior to interview
Prescribing authority or ability, or ability to obtain prior to employment
DEA license, or ability to obtain prior to employment
Board certified or Board Eligible

Preferred Qualifications:
1+ year professional experience in Family Practice, Pediatrics, and/or Urgent Care
