Registered Nurse RN / MDS Coordinator / Assisted Living

Job Responsibilities

The objective of MDS coordinators is to promote the physical and emotional well-being of nursing facility residents. They use a Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic interviews. The MDS covers such areas as the patient's mood, behavior patterns, cognitive ability, and nutrition needs.

Additional responsibilities for the MDS Coordinator RN include:

Manages the overall process for tracking all the Medicare documents in order to assure appropriate reimbursement for services provided
Conducts concurrent MDS reviews to assure achievement of the maximum allowable RUG categories
Updating and monitoring resident ICD-9 coding
Regulates and maintains the MDS/PPS program and completion schedule
Develops and coordinates appropriate nursing care plan from MDS/RUG category
Attend and contribute necessary information during the Medicare meetings
Identify residents with significant changes after MDS review and refer appropriately
Analyze assessment data independently for complex resident problems, early subtle changes that formulate RUG categories and effect resident care
Work collaboratively with the Rehab Manager to identify appropriate ARD dates for optimal reimbursement reflecting the care resident receive

Job Requirements

We are looking for dedicated and professional MDS Coordinator RN who is committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.

Specific qualifications for the MDS Coordinator RN role include:

Must be an RN or LPN
One to three years of experience working in long term care.
Experience working with persons with dementia preferred
Ability to communicate effectively verbally and in writing
Excellent interpersonal and communication skills
Clear understanding or person centered care
