BASIC FUNCTION: The purpose of the Case Manager II position is to support the physician, primary medical homes, and interdisciplinary teams in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The role integrates and coordinates resource utilization management, care facilitation and discharge planning functions. In addition, the Case Manager II helps drive change by identifying areas where performance improvement in needed (e.g., day to day workflow, education, process improvements, patient satisfaction). The position is responsible for coordinating a wide range of self management support and provides information to update and maintain relevant disease registry activity.

The Case Manager II is accountable for a designated patient caseload and plans effectively in order to meet patient needs across the continuum, provide family support, manage the length of stay, and promote efficient utilization of resources. Specific functions within this role include:
Facilitation of precertification and payor authorization processes
Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement
Application of process improvement methodologies in evaluating outcomes of care
Support and coaching of clinical documentation efforts and serving as a clinical resource for coders ensuring that documentation accurately reflects severity of illness and intensity of service
Coordinating communication with physicians.

Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.

MINIMUM QUALIFICATIONS, EDUCATION AND EXPERIENCE):
1. Current and valid license to practice as a Registered Nurse (ADN or BSN) in the state of Texas
2. Case Manager Certification required
3. Three or more years strong clinical experience in clinical practice area to which assigned
4. Two or more years of Case Management required
5. Excellent interpersonal communication and negotiation skills.
6. Demonstrated leadership skills.
7. Strong analytical, data management and PC skills.
8. Current working knowledge of discharge planning, utilization management, case management, performance improvement, disease or population management and managed care reimbursement.
9. Understanding of pre-acute and post-acute venues of care and post-acute community resources, physician office routines, and transitional procedures for pre and post acute care.
10. Demonstrated understanding of motivational interviewing and change management.
11. Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
12. Ability to work independently and exercise sound judgment in interactions with physicians, payors, and patients and their families.
13. Demonstrates commitment to the Partners-in-Caring process and the Behavioral Expectations in all interactions and in performing all job duties. Performs duties in a manner to promote quality patient care and customer service/satisfaction, while promoting safety, cost efficiency, and a commitment to the CQI process.
