Job Description

POSITION SUMMARY:

Reporting to the Clinical Care Manager, the CNA, Hospice Aide is responsible for assisting with direct patient care within the scope of their practice. Provides personal care to the patient and assists with activities of daily living in the patient's home, facilities and/or Inpatient units. The CNA is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an advocate for patients/families and their profession. Demonstrates knowledge of age-specific patient populations and cultural diversity during the provision of patient care. Contributes to the provision of quality care through performance improvement techniques that demonstrate positive outcomes in patient care. Promotes an environment of care. Supports the organizational vision and mission.

EDUCATION AND QUALIFICATIONS:

  • Current certification in the State of Florida as a Certified Nurse Assistant, with a minimum of one (1) year of experience preferred.
  • Must maintain current CPR certification.
  • Experience in caring for the terminally ill desired.
  • Experience caring for others with life threatening illness preferred.

ESSENTIAL JOB RESPONSIBILITIES:

  • Performs or assist patients with tub/shower/bath, complete/partial bed bath, personal care, skin care dressing, nail care, hair care, foot care and oral hygiene.
  • Take and records B/P, pulse, respiration, weight and temperature. Reports abnormalities to the RN.
  • Assists patient or caregiver with the use of Hoyer lift, commode, bedpan, urinal, wheelchair, hospital bed and other equipment as directed by registered nurse.
  • Assists patients to achieve maximum independence in their activities of daily living and promotes optimal well being for the patient through maintaining a clean, safe, and healthy environment.
  • Make beds; Change linens.
  • Assists with positioning, transfers, ambulating, and activity as directed by the registered nurse.
  • Demonstrates knowledge and practice of universal precautions. Follows established Infection Control and Universal Precautions policies and procedures when performing daily tasks.
  • Utilizes proper body mechanics while lifting and moving patients, and during any other activities.
  • Records patient's health status and services provided each visit in an informative and descriptive manner.
  • Immediately reports all changes in the patient's condition, all accidents and incidents to the Case Manager or NCM.
  • Reports all complaints and grievances made by the patient to the Case Manager or Clinical Supervisor.
  • Actively participates in interdisciplinary group (IDG) by informing RN Case Manager of changes/status.
  • Maintains a professional relationship and serves as a liaison with other disciplines.
  • Offers emotional support to the patient/family in accordance with the hospice philosophy and plan of care.
  • Represents Gulfside at organizations requesting CNA’s in the community.
  • Observe Patient Rights.
  • Treats all everyone fairly, with kindness, dignity and respect.
  • Maintains a positive attitude and caring spirit. Displays a “can do” attitude.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.

SKILLS AND COMPETENCIES:

  • Maintains up to date skill sets as required to care for patients.
  • Possesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Healthcare Services objectives.
  • Ensures compliance with all patient care and documentation standards and regulations.
  • Assists in the implementation and maintenance of the policies and procedures of the clinical services department; adheres to and assists in the evaluation of the same.
  • Assists in maintaining federal and state regulatory compliance.
  • Proficient in Electronic Medical Records, emails and basic computer knowledge.

  • While performing the duties of this job, the employee may be required to stand, walk, sit, use hands to finger, handle or feel objects/tools/controls, reach with hands/arms, climb stairs, balance, stoop, kneel, crouch or crawl, talk, hear, taste, or smell. This employee must occasionally lift and/or move from 10 to 250+ pounds. Specific vision abilities required for this position include close vision, distance vision, color vision, depth perception, and the ability to adjust focus.
