Long-Term Care / Rehabilitation
Position Summary
The Certified Nursing Assistant (CNA) provides direct patient care and assists residents with activities of daily living (ADLs) under the supervision of a licensed nurse. CNAs support residents’ physical, emotional, and social needs in a long-term care and rehabilitation environment.
Essential Duties & Responsibilities

Assist residents with ADLs including bathing, dressing, grooming, toileting, and feeding

Support mobility: transfers, ambulation, positioning, and use of assistive devices

Monitor and report changes in resident condition to nursing staff

Take and record vital signs, intake/output, and daily care documentation
-
Assist with rehabilitation exercises and therapy activities as directed
-
Maintain a clean, safe, and comfortable environment for residents
-
Respond promptly to call lights and resident requests
-
Follow infection control, safety, and HIPAA guidelines
-
Provide compassionate, respectful, and patient-centered care
-
Assist with admission, discharge, and transport of residents as needed
Qualifications
-
Current CNA certification in good standing (state-approved)
-
High school diploma or equivalent preferred
-
Ability to work in a fast-paced LTC/Rehab setting
-
Strong communication and interpersonal skills
-
Ability to lift, push, pull, and assist residents safely
