Personalcare Registered Professional Nursing P.C. is looking for dedicated and hard-working Peer Recovery Support Specialist to work full-time on the day shift in the medical department at the Monroe County Jail located in Rochester, NY.

Available Shift: FT Day (Sunday through Thursday, 7:00 AM - 3:30 PM)

Responsibilities:

• Responsible for introducing new inmates to the process at Monroe County Jail upon entry/re-entry. To provide peer support from personal gained experiences. To assist in discharge and re-entry planning.
• Provide initial, non-clinical contact to individuals entering the facility to provide support
• Engage with individuals entering to determine level of support needed by both medical, mental health, and rehabilitative services provided.
• Provide support to individuals entering and/or re-entering the facility as it relates to experience, feelings, and process of current circumstances
• Coordinate care with appropriate teams to ensure individual is properly and effectively connected with services requested at point of intake.
• Work closely and collaboratively with Transitional Coordinator to meet with individuals to gather proper information for re-entry/discharge planning.
• Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines.
• Utilize the electronic medical record upon each visit and document properly to reflect services provided.
• Service as a model of hope, recovery, and stability utilizing lived and shared experience(s)

Hourly Rate: $17.00 to $21.00
