Location: Near Concord, NH
Schedule: Multiple shift options; Evening (12:00 PM–10:00 PM) or Overnight (11:00 PM–7:00 AM)
Job Type: Permanent, Full-Time
Pay: $38–46 per hour (DOE)

Overview:
Our medical practice near Concord is seeking a Licensed Practical Nurse (LPN) to support clients through withdrawal management. New grads and experienced LPNs alike are welcome to apply.

Key Responsibilities:

• Perform intake assessments, including health histories and symptom evaluation
• Conduct Clinical Opiate Withdrawal Scale (COWS) and Clinical Institute Withdrawal Assessment (CIWA) assessments
• Provide medical monitoring of clients in withdrawal, recording vital signs and reporting changes promptly
• Administer medications safely, following practice protocols and provider orders
• Support individualized care plans in collaboration with RNs and other healthcare professionals
• Educate clients and families on treatments and next steps
• Document interactions, assessments, and interventions in the electronic health record (EHR)
• Participate in interdisciplinary care conferences

Qualifications:

• Active, current Licensed Practical Nurse (LPN) licensure in New Hampshire
• Comfortable performing withdrawal assessments and medical monitoring
• Strong communication and patient education skills
• Experience in a hospital, clinic, or long-term care setting preferred
