Key Responsibilities:

• Administer medications and treatments as prescribed
• Monitor residents and report changes in condition
• Assist with assessments and implementation of care plans
• Maintain accurate and timely documentation
• Collaborate with RNs and interdisciplinary care teams
• Deliver compassionate, resident-centered care

Requirements:

• Active LPN license in New York
• 1+ year of experience in Long-Term Care, Skilled Nursing, or Rehab preferred
• Strong clinical and communication skills
• Ability to work in a fast-paced environment
