Job Description
Specialty: LVN/LPN – Clinic Setting
Duration: 13 wk
Shift: M-F 10a-6p
Pay: $40/hr
Start: ASAP
ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following. Other duties may be assigned.
- Communicates with health care providers and care team in providing care for each patient . If necessary, may assist with interpretation to aid the provider with the patient visit.
- At the direction of the RN, functions as a Case Manager by facilitating the delivery of individualized and coordinated care.
- Identifies patient goals for consideration by the RN for possible inclusion in the patient care plan in the management of complex care patients.
- Administers injections, medications, and performs treatments as per physician orders and UHC policies and procedures.
- Directs and assists in the opening and closing of the clinic including daily clinic assignments and preparing, rooming and exiting patients for provider visits as needed.
- Reports to provider and documents in the EMRpatients’ response to treatment and health education.
- Responds to desktop messages timely and in collaboration with members of the Health Care Team.
- Maintains availability and readiness of all equipment and supplies in assigned areas.
- Assists in orientation of nursing staff including Medical Assistants and LPN’s.
11. Assists in the coordination of patient flow within the clinic.
12. Coordinates response to patient emergencies and initiates immediate action.
13.Supports and reinforces policies regarding patient care, infection control and safety.
14.Participates in Quality Assurance activities.
15.Complies with the Union Health Center’s policies & procedures related to privacy and security of identifiable patient information (PHI), as explained in the HIPAA (Health Insurance Portability & Accountability Act) training.
SUPERVISORY RESPONSIBILITIES
Works in collaboration with Team Supervisors to supervise medical assistants when providing patient care.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The Union Health Center will comply with applicable standards of the Americans with Disabilities Act.
1. New York State Licensed Practical Nurse.
2. Two years of ambulatory experience as an LPN preferred.
3. Strong interpersonal, oral, written communication and collaborative skills.
4. Computer literate. Knowledge of EMR a plus.
5. Bilingual English/ Spanish required.
6. Covid Vaccination is required.
