Job Description:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team in Jacksonville, FL. The ideal candidate will work collaboratively with our care team, including the Physician Assistant, Nurse Practitioner, and Psychiatrist, to deliver coordinated medical and psychiatric care to patients.

Key Responsibilities:
- Administer medications and treatments as prescribed by the healthcare team.
- Assist in the development and implementation of patient care plans.
- Monitor patient health by assessing vital signs and providing basic patient care.
- Support the care team in delivering coordinated medical and psychiatric care.
- Document patient information accurately and maintain detailed records.
- Communicate effectively with patients, families, and the healthcare team to ensure high-quality care.
- Educate patients and families on treatment plans, medications, and preventive care techniques.
- Ensure compliance with all health and safety regulations.

Qualifications:
- Active Florida Licensed Practical Nurse (LPN) license.
- At least six months of nursing experience as an LPN, or two years of patient care experience as a Certified Nursing Assistant (CNA).
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a team-oriented environment.
- Excellent organizational and time-management skills.
