Licensed Practical Nurse

We are seeking a qualified and dedicated Licensed Practical Nurse (LPN) to join our team, a long-term adult care facility. The ideal candidate will possess a strong commitment to providing high-quality healthcare services and have the ability to manage medication, conduct initial intakes, and perform other LPN duties efficiently. Key Responsibilities:
- Conduct initial patient intakes and assessments.
- Manage and administer medications as prescribed.
- Monitor patient health and report any changes to the healthcare team.
- Assist in the development and implementation of patient care plans.
- Collaborate with healthcare team members to ensure patient safety and well-being.
- Provide basic patient care, including bathing, dressing, and feeding.
- Maintain accurate and up-to-date patient records.
- Educate patients and their families on health maintenance and disease prevention.
- Perform routine checks on medical equipment and ensure its proper functioning.

Qualifications:
- Valid LPN license from any U.S. state.
- Minimum of 1 year of experience as a Licensed Practical Nurse.
- Current BLS/CPR certification.
- Ability to pass a background check, credit check, and drug screening.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Detail-oriented with excellent organizational skills.
