Job Responsibilities:
- Administer medications and vaccinations as prescribed, including controlled substances, in accordance with facility protocols
- Monitor and document patient vital signs, symptoms, and responses to treatment
- Provide basic wound care, assist with specimen collection, and support chronic disease management
- Assist with intake screenings, daily sick calls, and emergency response situations
- Prepare and maintain accurate patient records in the Electronic Health Record (EHR) system
- Communicate effectively with providers, RNs, mental health staff, and custody personnel to coordinate patient care
- Maintain infection control standards and ensure proper handling and disposal of medical waste
- Support patient education related to medication adherence, hygiene, and overall wellness
- Monitor medical supply levels and report needs to supervising staff
- Maintain professional boundaries and comply with all facility rules and security protocols

Qualifications:
- Active and unrestricted LPN license in the state of Pennsylvania
- LPN experience preferred; correctional, urgent care, or behavioral health experience is a plus
