Job Description

Key Responsibilities:
- Administer medications and vaccinations per facility protocols
- Monitor vital signs, conduct assessments, and assist with resident intakes
- Provide basic wound care and support chronic disease management
- Assist with Activities of Daily Living (ADLs)
- Document care accurately in the Electronic Health Record (EHR)
- Collaborate with Registered Nurses (RNs), providers, and care team members
Qualifications:
- Active, unrestricted LPN license in California
- LPN experience preferred
- Current BLS/CPR certification
