Job Summary
We are looking for a reliable and compassionate Licensed Practical Nurse (LPN) to join our Brooklyn team. In this role, you will provide high-quality care, support residents with daily living needs, and work closely with a multidisciplinary team to promote health and wellness.

Pay & Benefits:
- Salary: $70,720–$79,040 per year (depending on experience)
- Full-time schedule

Responsibilities:
- Administer medications and vaccinations
- Monitor resident health and record vital signs
- Provide wound care and support chronic disease management
- Assist with daily activities such as bathing, dressing, feeding, and mobility
- Document care accurately in the electronic health record (EHR)
- Educate residents on medications, hygiene, and wellness
- Collaborate with healthcare staff to coordinate care
- Follow infection control and safety protocols
- Participate in screenings, sick calls, and emergency response

Qualifications:
- Active and unrestricted LPN license in any U.S. state
- Previous LPN experience preferred; correctional, urgent care, or behavioral health experience is a plus
- Strong communication and interpersonal skills
- Ability to provide culturally sensitive, patient-centered care
