Key Responsibilities:

  • Administer medications, injections, and vaccinations in accordance with clinical protocols
  • Monitor patient conditions, vital signs, and responses to treatment
  • Assist with patient intake, screenings, and routine clinical support
  • Perform basic wound care and assist with specimen collection
  • Support patient education and chronic disease management initiatives
  • Maintain accurate and timely documentation within electronic medical records
  • Respond appropriately to urgent patient care situations and clinical needs
  • Collaborate with providers and interdisciplinary teams to ensure quality patient outcomes
  • Follow infection control, safety, and regulatory compliance standards

Qualifications:

  • Active and unrestricted Licensed Practical Nurse (LPN) license in Pennsylvania
  • Current BLS certification or ability to obtain upon hire
  • Previous LPN experience in a healthcare or clinical setting preferred
  • Experience in correctional health, urgent care, behavioral health, or primary care is a plus
  • Strong communication, organizational, and patient care skills
  • Experience with electronic health record (EHR) systems preferred
  • Ability to thrive in a fast-paced, team-oriented healthcare environment
