Overview:
Our healthcare team in Brooklyn seeks a reliable and compassionate Licensed Practical Nurse (LPN) to provide high-quality resident care and support daily health needs. Our LPNs collaborate with a multidisciplinary team to promote wellness, safety, and continuity of care. This role is suited for professionals committed to patient-centered and culturally sensitive care.

Position Offers:

• Full-Time Schedule
• Competitive Compensation: $70,720–$79,040 per year (DOE)

Key Responsibilities:

• Administer medications and immunizations as prescribed
• Monitor resident conditions, record vital signs, and report changes
• Provide wound care and support chronic condition management
• Assist residents with activities of daily living, including hygiene, mobility, and feeding
• Document all care accurately in the electronic health record (EHR)
• Educate residents on medications, wellness, and self-care practices
• Collaborate with clinical staff to coordinate individualized care plans
• Follow infection control protocols and respond to clinical emergencies as needed

Qualifications:

• Active and unrestricted LPN license in any U.S. state
• Prior LPN experience preferred; background in urgent care, behavioral health, or corrections is a plus
• Strong communication and interpersonal skills
• Ability to deliver culturally sensitive, patient-centered care
