Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) for a quick-start contract at Homewood at Plum Creek. This is a part-time role within a high-quality skilled nursing facility (50-149 beds) focused on geriatric care.
Schedule
Shift: Evening (3:00 PM – 11:00 PM)
Shift: Evening (3:00 PM – 11:00 PM)
- Commitment: 2 days per week + every other weekend
- Holiday Coverage: Required
Key Responsibilities
- Provide high-quality nursing care to residents in a Long-Term Care/Skilled Nursing environment.
- Utilize MatrixCare and Meditech EMR systems for accurate documentation and charting.
- Collaborate with the interdisciplinary team to maintain resident safety and comfort.
Requirements
- Experience: Minimum 1 year of LPN experience in Long-Term Care (LTC) or Skilled Nursing (SNF).
- Technical: Proficiency with MatrixCare is strictly required.
- Licensure: Must hold an active, current Pennsylvania LPN license.
- Urgency: This is a "Quick Start" position; candidates must be ready to begin by May 11.
