Job Title: Licensed Vocational Nurse (LVN) – Assisted Living / Memory Care
Location: Wichita Falls, TX

Overview:
Our rapidly growing Memory Care and Assisted Living expansion offers an opportunity for an LVN to play a direct role in shaping clinical operations. Our privately owned setting provides streamlined decision-making, strong leadership support, and meaningful clinical autonomy. This role blends hands-on nursing with care coordination in a stable, growth-focused environment.

Position Offers:

Monday–Friday schedule with flexible start and end times based on facility needs
Light rotating weekend/on-call phone support

Competitive Compensation: $60,000 – $75,000 annually (DOE) with performance bonuses at 6 and 12 months

Key Responsibilities:

Provide direct LVN nursing care in assisted living and memory care settings
Perform resident assessments, care planning, and ongoing clinical evaluations
Administer medications and ensure regulatory compliance across care delivery
Monitor resident status and respond to changes in condition appropriately
Coordinate CNA staffing coverage and support daily care operations
Assist with admissions, transitions, and continuity of resident care
Document all clinical care accurately within the facility EMR system
Support development and implementation of Memory Care unit processes

Qualifications:

Active Texas LVN license
1+ year of experience in geriatrics, long-term care, skilled nursing, assisted living, or memory care preferred
Strong medication administration and clinical care experience
Experience with EMR documentation systems preferred
Strong communication, organization, and independent work skills
CNA delegation or shift coordination experience is a plus
