FULL TIME POSITION **INFUSION EXPERIENCE NEEDED NOW!**

Summit Home Care is searching for a dedicated full-time Licensed Practical Nurse (LPN) to join our team for the Downtown Cincinnati and surrounding areas! The position comes with excellent pay and benefits package!

The ideal candidate is a passionate LPN with an unrestricted license and minimum 1 year of clinical experience. The candidate will exercise independent and sound judgement, has strong decision-making skills and well-developed interpersonal skills.

Our candidate must also have the ability to case manage multiple priorities, effective organizational and time management skills along with strong teamwork skills. Candidate must possess good computer skills.

EDUCATION

The minimum level of education desired for candidates in this position is an Associate's degree. Advanced certification or willingness to earn advanced certification preferred.

JOB RESPONSIBILITIES

As a Home Health Nurse you will perform home visits to assess the patient’s health problems and needs as well as develop and implement a nursing care plan. You will be responsible for delivering quality medical care in compliance with industry/company regulations, policies, and procedures. You will also be responsible for active case management of your case load.

ADDITIONAL RESPONSIBILITIES

Maintaining accurate, detailed reports and records using computerized documentation

Administering medications to patients and monitoring patients for reactions or side effects

Modifying patient treatment plans as indicated by patients' responses and conditions

Monitoring all aspects of patient care, including diet and physical activity

Instructing patients and families on topics such as health education or disease prevention

Be able to work in a fast-paced environment
