LPN - Adult/Home Care - Souderton, PA

Full-time and part-time employment available

We are founded on the belief that people living with intellectual and developmental disabilities are not defined by their diagnosis and, with the proper support, are able to achieve their dreams and become valued members of their communities. Currently seeking dedicated LPNs to work in our home base setting with an individual with complex medical needs.

Shifts:

Full-time: 8am-8pm

Part-time: 2pm - 10pm

Qualifications and Education Requirements:

  1. Current unencumbered nursing license (LPN) in state of PA
  2. Current CPR
  3. Minimum 1 year LPN home care experience
  4. Graduate of an accredited nursing program

Required Skills:

  • Excellent Documentation skills
  • Ability to complete head to toe assessment and communicate findings
  • Proficient in G-Tube and J-Tube care and medication administration
  • Ability to perform standing changes with individual
  • Knowledge of respiratory treatments such as inhalers and nebulizers.
  • Use of appropriate policies and systems as required for this position.
  • Basic understanding of non-verbal communication skills and understanding deviations from individual’s baseline affect.
  • Compassionate, caring, and punctual

Preferred Skills:

  • Communication Skills (written and verbal)
  • Computer skills/data entry
  • Knowledge of 6400 (CLA) and 6500 (Life Sharing) PA ODP Regulations
  • High degree of willingness to learn, ability to change and be flexible based on situational requirements
  • Organizational Skills and Multi-tasking
  • Attention to detail
  • Reliable and able to meet deadlines with minimal prompting
  • Person-centered thinking experience or training
  • Creative

Duties and Responsibilities:

  • Ensure the well-being of the consumer.
  • Daily nursing assessments and monitoring of both individuals living in the home ongoing health.
  • Provide appropriate care in accordance with personal centered plan and all physician order.
  • Provide home care nursing to adult consumer in home and community setting.
  • Take individual out in the company van to community-based activities
  • Assist individual in participating in meaningful activities
  • Create a self-centered environment for the individual - focused on his/her hopes, dreams, and goals.
  • Assist the individual in completing daily objectives related to their person centered goals.
  • Follow all care plans, ISP, and protocols.
  • Ensure that individual’s home and medical equipment is clean and safe.
  • Daily completion of individual’s personal hygiene related tasks
  • Lead for insuring individual’s health maintenance
  • Accompany individual to appointments and report findings to program specialist
