Overview:
We are seeking a Licensed Practical Nurse to serve as a Care Navigator in an independent living community in Mission, KS. This role provides direct patient support focused on urgent and preventative care in a low-volume, autonomous clinical setting. The ideal candidate is self-directed, organized, and skilled in building strong patient relationships.

Key Responsibilities:

Provide nursing support as a Care Navigator within an independent living community
Deliver urgent and preventative care to residents
Perform patient assessments, care coordination, and follow-up
Collaborate with interdisciplinary teams to support patient wellness
Build strong patient relationships through clear and compassionate communication
Maintain accurate and timely documentation of all care provided
Perform phlebotomy and basic clinical procedures as needed
Identify changes in patient condition and escalate appropriately

Qualifications:

Active Licensed Practical Nurse (LPN) license in the state of Kansas
Strong phlebotomy skills required
Ability to work independently as the primary clinical staff on site
Strong organizational and communication skills
Geriatric experience preferred but not required
Open to new graduates or experienced LPNs
