As a Licensed Vocational Nurse (LVN) in Home Health & Hospice, you will work under the direction of a Registered Nurse (RN) or Medical Doctor (MD) to provide high-quality general nursing services, manage patient treatments, and support interdisciplinary care plans.

Key Responsibilities

• Direct Patient Care: Provide direct nursing care for assigned hospice and home health patients in accordance with established policies and individual care plans.
• Clinical Procedures: Perform routine visits and clinical procedures including phlebotomy, injections, drug screenings, sterile dressings, irrigations, and cauterizations using strict aseptic techniques.
• Medication Administration: Safely administer medications as prescribed and monitor patient response.
• Patient & Family Education: Implement individual teaching plans and discharge plans focused on patient decision-making and comfort care.
• Documentation & Coordination: Utilize EPIC EHR to accurately document visits, coordinate care with the interdisciplinary team, and report patient updates.
• Operations & On-Call: Provide functional direction to ancillary personnel, restock clinical supplies, and participate in on-call duties as assigned by agency operational needs.

Qualifications & Job Requirements

Mandatory Criteria (Must Have)

• EPIC EHR Experience: Demonstrated proficiency using EPIC software in a healthcare setting.
• Hospice Experience: Recent clinical experience in hospice or palliative care.
• Licensure: Active, unencumbered Licensed Vocational Nurse (LVN) license in the State of California.
• Certifications: Current Basic Life Support (BLS) Healthcare Provider certification from an approved vendor.
• Licensing & Driving: Valid Driver’s License (DL), age 21 or older, with a reliable vehicle and clean driving record.

Preferred Qualifications

• High School Diploma/GED required; Bachelor’s degree or equivalent combination of education and experience preferred.
• Arrhythmia certification preferred.

Compensation & Benefits

• Pay Rate: $40.00/hr on W2
• Shift Structure: Consistent 40-hour work week (5 x 8-hour shifts).
