Overview:
Our private duty home care team in Detroit is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for a patient in their home. This part-time role offers a consistent every-other-weekend schedule and the opportunity to build a meaningful, ongoing patient relationship.

Position Offers:

• Schedule: Every other weekend (Saturday–Sunday), 7:00 AM–7:00 PM
• Competitive pay: $30 per hour (DOE)

Key Responsibilities:

• Provide direct, hands-on nursing care to a private duty patient in their home
• Monitor and document vital signs, including hourly blood pressure checks, and report changes to the supervising RN or provider
• Administer medications per the plan of care and maintain detailed medication administration records
• Provide ongoing suprapubic catheter care and monitor for any concerns
• Document care, observations, and interventions in the Electronic Health Record (EHR)
• Educate the patient and family on care needs and treatment plans
• Assist with activities of daily living, meal preparation, and light housekeeping while promoting the patient's independence
• Provide transportation via the patient's handicap-accessible van to medical appointments and other activities

Qualifications:

• Active, unrestricted LPN license in Michigan
• Minimum 2 years of LPN experience
• Home health or private duty experience preferred
• Valid driver's license with a clean driving record
• Current Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification
• Comfortable working in a home environment with household pets
