Job ID: 131009 Direct Hire Licensed Practical Nurse (LPN) Role $28-$36 per hour in Charlottesville, Virginia Clinical Magnet (part of ICON Medical) is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our partner's Home Care team in Charlottesville, Virginia. This is a full-time, direct hire opportunity with a supportive team and a meaningful mission: delivering care to patients in their homes. Position Highlights: Schedule: Monday - Friday, 8:00 AM - 5:00 PM (with weekend rotation) Compensation: $28.85 - $36.06/hr, based on experience Start Date: ASAP Employment Type: Full-time, Direct Hire Location: Charlottesville, Virginia (must have reliable transportation) Reports to: Kim (Recruitment Contact) Key Responsibilities: Deliver general nursing care, including administering medications and treatments per physician orders Provide in-home services and follow assigned care plans in compliance with regulations and agency policy Conduct diagnostic and preventative procedures, including phlebotomy and vaccinations Educate participants and families on medications, treatments, and care instructions Maintain thorough and accurate documentation of all care provided Monitor patient responses and report conditions or changes to physicians and RNs Coordinate with interdisciplinary teams to implement and adjust care plans Participate in quality assurance activities, peer reviews, and care planning meetings Supervise and collaborate with healthcare personnel including CNAs Perform duties both at client homes and within agency facilities as needed Requirements: Active LPN License in the state of Virginia Current First Aid and BLS Certification (American Heart Association or Red Cross accepted) Valid driver's license, personal transportation, and auto insurance High School Diploma or GED Preferred Qualifications: 3+ years of LPN experience in a clinic or long-term care setting Experience working with geriatric patients Familiarity with medications commonly used in elderly care If you're a compassionate LPN ready to make an impact in home health, we'd love to hear from you!
