Job Responsibilities:
- Administer medications as prescribed and monitor patients for adverse reactions or side effects.
- Provide direct patient care, including wound care, personal hygiene, and assistance with activities of daily living.
- Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature.
- Manage pain and symptoms effectively under the supervision of a Registered Nurse (RN).
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Educate patients and families about the disease process, palliative care, and end-of-life issues.
- Document patient care accurately and timely in accordance with company policies and regulatory requirements.
- Participate in case conferences and communicate effectively with the healthcare team.
- Maintain a safe and comfortable environment for patients and families.

Qualifications:
- Active LPN license in good standing with the State of Indiana.
- Experience and knowledge in hospice, palliative, and geriatric end-of-life care.
- Skilled in medication administration, symptom management, and direct patient care.
- Exceptional communication and interpersonal skills, with the ability to support patients and families with sensitivity.
- Reliable transportation and a valid driver's license for travel between patient locations.
