Position Summary

We are seeking an experienced and compassionate Licensed Vocational Nurse (LVN) with recent hospice experience to provide skilled nursing care to patients in a home hospice setting. Under the supervision of a Registered Nurse (RN) or Physician (MD), the LVN will participate in patient assessments, implementation of care plans, and delivery of clinical treatments while ensuring high-quality, patient-centered care.

The ideal candidate will have strong clinical skills, hospice experience, EPIC documentation experience, and flexibility to work weekends along with three weekdays.

Key Responsibilities

  • Provide skilled nursing care to assigned hospice patients under the direction of an RN.
  • Perform routine patient visits according to agency policies and care plans.
  • Assist in developing and implementing individualized interdisciplinary plans of care.
  • Monitor patient conditions and report changes to the supervising RN or physician.
  • Administer treatments and nursing procedures using proper aseptic and sterile techniques.
  • Perform wound care, sterile dressing changes, irrigations, and other prescribed treatments.
  • Accurately document patient care and clinical observations in EPIC.
  • Educate patients and family members regarding disease management and comfort care measures.
  • Coordinate care with physicians, nurses, therapists, and other healthcare professionals.
  • Provide guidance and functional direction to ancillary staff when appropriate.
  • Participate in on-call responsibilities as assigned based on agency operational needs.

Qualifications

Required

  • Current Licensed Vocational Nurse (LVN) license in the state of practice.
  • Recent Hospice Nursing Experience.
  • EPIC EMR Experience.
  • Basic Life Support (BLS) Certification from an approved provider.
  • Ability to work weekends and additional weekday shifts.
  • Strong communication, documentation, and patient care skills.

Preferred

  • High School Diploma/GED.
  • Bachelor's Degree or equivalent combination of education and related experience.
  • Arrhythmia Certification.
  • Valid Driver's License and reliable transportation.
  • Age 21 years or older.

  • 40 guaranteed hours per week
  • Weekend coverage required
  • Three additional weekday shifts
