Job Description
LPN - PRN – Care Services (Admissions)
Assists in following our existing patients in hospitals. Facilitates discharges to Hope Care Centers (Hospice Houses), Patient Home, or Facility, provides teaching to patients to decrease hospitalizations, educates hospital staff of Hope’s programs and services, and assists our RN’s with admission processes. Must be willing to travel to all hospital locations. Computer skills and experience required. Days and Hours will vary - scheduled on an as needed basis.
