Start Date: Typical start date is within approximately 2 weeks after offer acceptance and completion of onboarding/credentialing requirements.
Experience Requirements
- Minimum 1 year of LPN/LVN experience
- Long Term Care (LTC) experience preferred
- Experience administering medications and treatments
License & Certification Requirements
- Active Colorado LPN/LVN License
- BLS/BCLS Certification

Job Summary
The Licensed Practical Nurse (LPN) provides direct and indirect nursing care to residents in a Long Term Care setting. The LPN is responsible for medication administration, treatments, resident assessments, documentation, communication with physicians and families, and supervision of CNA staff.
Key Responsibilities
- Provide direct resident care during assigned shifts
- Administer medications and treatments as ordered
- Monitor resident conditions and report changes appropriately
- Complete nursing documentation accurately and timely
- Document acute illnesses, physician visits, passes, and returns
- Communicate updates, changes, and new orders to MDPOA as required
- Supervise and direct CNAs providing resident care
- Promote teamwork across departments and shifts
- Participate in meetings and educational programs
- Maintain regular attendance and punctuality
