Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) with Long-Term Care and Skilled Nursing experience for a 13-week travel assignment. The ideal candidate will provide compassionate, high-quality patient care while collaborating with physicians and the interdisciplinary healthcare team.
Key Responsibilities
- Administer medications and treatments as prescribed.
- Perform patient assessments and monitor resident conditions.
- Assist with care planning and implementation.
- Maintain accurate and timely documentation in the EMR system.
- Coordinate care with physicians and other healthcare professionals.
- Provide resident-centered care in a fast-paced long-term care environment.
- Work assigned schedules, including weekends and holidays as needed.
Requirements
- Active Hawaii LPN License
- Current BLS/BCLS Certification
- Minimum 1 year of recent Long-Term Care or Skilled Nursing experience
- Experience using electronic medical records (EMR)
- Strong communication, organizational, and clinical assessment skills
- Ability to work independently and as part of a multidisciplinary team
